BASIC REQUIREMENTS

Original Philippine Passport

• Valid at least 6 months before return date

• Must have 2 blank pages

• Remove passport from holder

• Damaged passports not accepted

Japan Visa Application Form

• Computerized

• Printed on A4 paper

• With wet/physical signature

 

One (1) Passport Size Photo

• Size: 4.5cm x 3.5cm

• White background

• Taken within 6 months

1. Colored Photocopy of Passport Bio Page

2. Old Passport & Previous Japan/G7 Visas (if any)

 

PSA Documents

• Birth Certificate

• Marriage Certificate (if married)

 

If late registered:
Submit Baptismal Certificate + Form 137

 

If unavailable:
Provide signed Letter of Explanation.

FINANCIAL REQUIREMENTS

1. Bank Certificate with ADB/YTD

2. Latest 6 Months Bank Statement

3. ITR / Tax Documents
   (2316 / 1701 / 1702 etc.)

 

If unavailable:
Provide signed Letter of Explanation.

• PRC or IBP ID

 

WITH GUARANTOR

Additional requirements:

• Letter of Guarantee

• Proof of relationship

• Guarantor’s financial documents

• Submit at least 3 months before travel date

• No confirmed flight/hotel required

• Incomplete documents will not be accepted

• Visa approval is subject to Embassy assessment

• Visa fee is non-refundable
